IMACC MEMORIAL FUND SCHOLARSHIP FACULTY INFORMATION FORM
Philosophy: The IMACC scholarship is awarded annually to a student attending an Illinois two-year or four-year college meeting all of the eligibility criteria stated below and selected by the
IMACC scholarship committee. The scholarship is awarded in memory of James Armstrong, a
University of Illinois Professor until his death. Mr Armstrong was instrumental in establishing
the Allerton Conferences and in fostering better relations between two-year and senior colleges.
The scholarship provides approximately $750 or more to the selected recipient to help defray the
costs encountered at an institution of higher education. The money may be used for whatever
propose the recipient deems necessary.
Eligibility Criteria: Students meeting all of the criteria may apply for the IMACC Scholarship.
If, in the opinion of the Selection Committee, There is no bonafide candidate, the scholarship
will not be awarded that year. The IMACC Board may elect at the spring meeting to carry over
the funds to the next year The decision of the Committee is final.
The applicant must be a citizen of the United States. The individual must have a cumulative grade point average of B or Better; he/she must be pursuing an education at some institution of higher learning in the State of Illinois with a high demonstrated potential to complete the program and must have earned at least 40 semester hours at a community college
An applicant must be intending to complete a degree in mathematics education (preferred), or mathematics.
Each applicant must be nominated by a current member of IMACC or faculty from an institutional member who must submit a statement of his/her rationale for nominating the applicant. This written statement must be sent to the Chair of the Selection committee under separate cover from that of the student application. The written nomination must not exceed one double-spaced typewritten page. Four copies are required. It is emphasized that the person nominating a student shall not assist the student in the writing of his/her personal statement.
The applicant must submit a written application and transcript of courses and grades. Details concerning career objectives, work experiences, financial need, general philosophy of education, and other information that is felt to be helpful in assisting the Selection Committee should be included. The written application must not be longer than five double-spaced standard typewritten pages. Four copies are required. These copies must be sent under separate cover to the Chair of the Selection Committee.
The deadline for receiving applications is March 1 of the school year.
The scholarship will be awarded to selected applicants after receiving a written verifications from the college records office that the applicant has enrolled in an institution of higher education in the State of Illinois as a full-time student. Full-time student status shall be decided by the institution requirements. Failure to do this will result in the disqualification of the applicant and the next eligible applicant may be awarded the scholarship.
The scholarship will be awarded to an individual student only once. The student may, however, have his/her name submitted a maximum of two times.

transcript of courses and grades to meet the March first deadline. The official transcript
must show at least 40 hours completed at the Community College. Four copies of this
information form and the written application are required and must be sent to the

The forms, application and a transcript must reach my office not later than March 1 of the school year. Allow five days for delivery. Grades for te courses your are taking this semester must ve on the official transcript if these courses are to be considered as part of the evaluation process.
